what might be the difference between these two sizes?

Call of Duty - Black Ops III ... 49.85 GB 
Call of Duty - Black Ops III ... 55.49 GB

Each DL is for the same file: "rld-caofdubl3.iso"

By the difference, I would say that one of them (55.49GB) already includes the Nightmare mode (zombie mode).

Well, without dling and having a peek at it, it's hard to tell, looking at the dates the 55.49GB ones are more recent, so I presume that they have Nightmare Mode included (the 49.85GB do not have it included) plus some updates and maybe the Russian language pack, but I might be wrong.

I would grab the 55.49GB.
